On you first appointment you should bring with you your insurance cards and medication/allergy list.  You should also come 15 minutes early to allow the staff time to prepare your chart for the doctor.  Below is some paperwork that you can fill out ahead of time.  You can print this paperwork and bring it with you on your first visit.

Small procedures can be done in the office on your first appointment if desired.  Our office is able to obtain X-rays on site.
